NEW PATIENTS

We are currently accepting new patient newborns and new patient transfers! To become a new patient, please complete the registration, new patient office policies and records transfer forms below and return to the office with a copy of your insurance cards. Once records are received and the primary care provider (PCP) is updated, we can schedule. 

Are you a new/expecting parent wanting your newborn to be seen at Hartman Pediatrics? Call to be added to our patient panel. 

*Please note the office does not accept patients who are unvaccinated or choose to delay vaccines. We follow the CDC and ACIP vaccine schedule. We do not follow alternate vaccine schedules.
