NEW PATIENTS

We are currently accepting new patient newborns and new patient transfers! To become a new patient, please complete the registration, financial policies and release of records forms below, return to the office with a copy of your insurance cards. Once records are received and the primary care provider (PCP) is updated, we can schedule. 

Are you a new/expecting parent wanting your newborn to be seen at Hartman Pediatrics? Call to be added to our patient panel once the baby is born.  

* Vaccination is one of the most important components of preventive health care in pediatrics. Please note the office does not accept patients who are unvaccinated or those who choose to delay vaccines. We do not follow alternate vaccine schedules.
